Abstract: 

Background: Gastric cancer is the fourth leading cause of cancer-related mortality worldwide. Objectives: This study describes the epidemiologic, pathologic, imaging, and surgical findings of patients with gastric cancer referred to a tertiary hospital in Tehran, Iran. Methods: This was a single-center cross-sectional study. All the patients referred for a staging LAPAROSCOPY of gastric cancer in a tertiary hospital in Tehran, Iran, were included. Results: A total of 30 patients participated in the study. The mean age of the patients was 62 ± 12.6 years, of whom 76.7% were male. None of the patients had a family history of gastric cancer. Four patients were active smokers, 1 had opium addiction, and 1 had a history of chronic alcohol use. Most patients (n = 29) had at least 1 symptom or were symptomatic for 2.8 months (2.8 ± 2.3 months). The most common clinical complaints were abdominal pain, weight loss, and nausea. Within the follow-up period, 43.3% (n = 13) of the patients expired. The Kaplan-Meier diagram of the study revealed a mean survival time of 8.4 ± 2.1 months within the 2-year follow-up, with a median of 6.0 ± 2.3 months. Conclusions: Our results confirmed the previous findings of the clinicopathologic characteristics of gastric cancer patients. Our primary findings were a younger mean age at the diagnosis, advanced stages of cancer at the first visit, low rates of comorbidity and risk factors, an overall survival rate of 43.3%, and a mean survival time of 8.4 ± 2.1 months in 2 years of follow-up.

Abstract: 

Background: Endometriosis has been widely implicated as one of the causes of chronic pelvic pain, dysmenorrhea and infertility. The purpose of our study was to define the presumed correlation between visual and histologic DIAGNOSTIC features of endometriosis based on the peritoneal findings identified in LAPAROSCOPY as a DIAGNOSTIC trial with a standardized technique. So, all the specimens including complete excised lesions suggestive of endometriosis and systematic biopsies of normal appearing pelvic peritoneum were analyzed. This study was performed in Mahdieh hospital and IRHRC1 between 2004 and 2005.Materials and Methods: A DIAGNOSTIC study of 30 patients (14 to 45 years) undergoing DIAGNOSTIC LAPAROSCOPY for the evaluation of chronic pelvic pain, infertility, dysmenorrhea and dyspareunia was carried out (average age was 28.6±5.14).All areas suggestive of endometriosis were excised and examined pathologically. Peritoneal biopsy specimens were obtained from areas of normal appearing peritoneum to rule out microscopic endometriosis.The positive predictive value, sensitivity, negative predictive value and specificity were determined for identified endometriosis versus the histological findings.Results: The mean prevalence of abnormalities visually consistent with endometriosis was 63% while 42% confirmed histologically. The positive predictive value was 42.1%, sensitivity 88.8%, negative predictive 90.9% and specificity 47.6% for visual versus histological diagnosis of endometriosis.Conclusion: A diagnosis of endometriosis should be established only after histologic confirmation.

Abstract: 

BACKGROUND AND OBJECTIVE: Cryptorchidism or absence of one or both of testes in the scrotum (undescended testes) is the most common urologic disorder in children. Two major complications in this condition are infertility and malignancy. Most DIAGNOSTIC imaging procedure is ultrasound, but in cases that testes are not seen in ultrasound, LAPAROSCOPY is very useful. The aim of this study was to assess DIAGNOSTIC value of ultrasonography and LAPAROSCOPY in undescended testes.METHODS: This DIAGNOSTIC study was performed on all patients with undescended testes in teaching hospitals of Babol University of medical sciences, Babol, Iran. All patients were referred to sonography, if testes were seen in sonography, these patients were candidate for open surgery but if testes were not seen in two different sonographies, the patients were candidate for LAPAROSCOPY. All information is gathered in questionnaire and then the results of sonography, open surgery and LAPAROSCOPY were compared together.FINDINGS: One hundred and eight patients with 131 undescended testes, aged between 1 and 42 years with mean age of 13.9±8.99 years were studied. 78.7% of patients had unilateral undescended testes and 21.3% had bilateral undescended testes. The unilateral undescended testes were left sided in 47% and right sided in 53%. Sensitivity and specificity of sonography for intraabdominal testes were 16% and 97%, respectively. Whereas, sensitivity and specificity of sonography for inguinal canal testes were 85% and 76%, respectively. Also, sensitivity and specificity of LAPAROSCOPY for intraabdominal and inguinal canal testes were 100%.CONCLUSION: The results showed that DIAGNOSTIC LAPAROSCOPY has a high sensitivity and specificity. When intraabdominal testes are not diagnosed by sonography, LAPAROSCOPY can be helpful.

Abstract: 

Introduction: The preferred method in anesthesia of the patients who are candidate for gynecology DIAGNOSTIC laparoscopic surgeries is general anesthesia (GA) usually, but in patients with any contraindication for GA, Spinal Anesthesia (SA) is used. This study was performed with aim to compare GA with SA technique for gynecology DIAGNOSTIC laparoscopic surgeries.Methods: This clinical randomized and prospective study was performed on 30 patients who were candidate for gynecology DIAGNOSTIC laparoscopic surgeries in 2012-2013. The subjects were divided in two groups of GA & SA (n=15 in each group). Data collection tools were checklist and observation. SPO2, MAP, RR, HR, ETCO2 were recorded preoperative and intraoperative. Nausea, vomiting and shoulder pain were evaluated in recovery, 2, 8 and 24 hours after surgery. Data were analyzed by SPSS software (version 15), and using T-test and Chi-square. P<0.05 was considered significant.Results: The changes of SPO2 and HR at the studied times were not significantly difference between two groups (p=0.141, p=0.531, respectively), and only EtCO2 after Inflation (p=0.015) and RR and EtCO2 during inflation (p=0.004, p=0.013, respectively) were significantly different between two groups. MAP in all the state in the SA group was lower except before inflation. The incidence of shoulder pain at 2, 8 and 24 hours after surgery had significant difference between two groups (p=0.009, p=0.035 and p=0.011, respectively); so that it was higher in the SA group. Nausea in the recovery had higher incidence in SA group (p=0.024).Conclusion: The use of SA for DIAGNOSTIC laparoscopic surgeries in women is safe and reliable and like as any other anesthesia method has benefits and disadvantages.
